A vibrant community of individuals passionate about the work they do! What You’ll Do Evaluate patient pre-admission screenings and post-admission physician evaluations for timely completion. Review Plans of Care to ensure they reflect patient progress, required goals, anticipated treatments, therapy intensity, estimated length of stay and discharge destination. Monitor Plans of Care for completion, accuracy and consistency with physician, therapy and nursing assessments. Partner with case management to ensure patient information meets regulatory requirements. Monitor physician visit documentation and verify that required medical, functional and patient progress elements are included. Validate that physicians document changes to a patient’s course of treatment. Coordinate monthly audits and develop action plans that improve processes and support compliance. Educate nursing, therapy and medical staff on documentation and regulatory requirements. Conduct additional medical record reviews and analyses as needed. Qualifications: Current licensure as a nurse or allied health professional required; RN preferred. Bachelor’s degree or allied health degree required as appropriate for licensure. Four years of experience within the licensed discipline required. Acute inpatient rehabilitation experience preferred. Strong written and verbal communication, active listening, critical thinking and time-management skills. Ability to analyze clinical information, make decisions, meet deadlines and work independently. Proficiency with Microsoft Outlook, Word and Excel
