Position Summary                                                                                                                           

The Crisis Center Admissions and Discharge Coordinator provides direct services to patients in the 23-Hour Crisis facility and the Residential Site for Crisis Stabilization, with an emphasis on admission and discharge planning and aftercare follow up. These services will be provided to high-risk adults with a diagnosis of substance use disorder or mental health. This job description does not suggest or imply that these are the only duties to be performed by this employee. This person will be required to follow other instructions and to perform other duties assigned by the Program Administrator. This position is part of the administrative leadership team within the crisis facility.
